The Connetquot Youth Association is proud to present its 2008 Fall Baseball/Softball Season. The fall season will consist of non-competitive mostly instructional divisions. Uniforms will consist of shirts only. There will be no raffles or candy to sell. The season will run for 7 weeks, starting on Saturday, September 6 th, 2008.

I/We the parents of the above named candidate for a position on a Connetquot Youth Association Baseball team, hereby give my/our approval to his/her participation in any and all Connetquot Youth Association activities. I/We assume all risks and hazards incidental to such participation including transportation to and from the activities; and I/We do hereby waive, release, absolve, indemnify and agree to hold harmless the Connetquot Youth Association Incorporated, the organizers, sponsors, supervisors, participants and persons transporting my/our child to or from activities, for any claim arising out of an injury to my/our child, whether the result of negligence or for any other cause, except to the extend and in the amount covered by accident or liability insurance. In addition I/We the parent do herby grant to the Connetquot Youth Association the right to photograph the above named player and use the photo and/or other digital reproduction of him/her or his/her physical likeness for publication processes, whether electronic, print or digital or electronic publishing via the internet.

Please printout the Registration (all documents) PDF file. This contains all of the forms and information you need to register your child for the Spring ball season. You must sign and return the Parents Code of Conduct, Candy Fundraiser and Registration form along with your check no later than February 6, 2008 to Connetquot Youth Association, PO Box 221, Ronkonkoma NY 11779. NOTE: If this is the first year your child is playing ball you must also include a copy of his/her birth certificate. |
